Born June 1, 1952, in Fond du Lac, Wis., the son of Walter and Gloria (nee Luby) Goldapske, he passed away Wednesday, Nov. 17, 2004, at his home in Buffalo Grove. Jeffrey is survived by his beloved partner, Diane Howland; brother, Rick Goldapske; and sister, Peggy (Steve) Cohen. In lieu of flowers, memorials may be made to the Alzheimer's Association of Greater Chicago, 4709 Golf Road, Suite #1015, Skokie, IL 60076.
